About This Position
Description
Tata Homecare Services is seeking a highly organized, compassionate, and detail-oriented Care Coordinator to oversee client services and caregiver assignments. The Care Coordinator plays a key role in ensuring high-quality, client-centered home care by coordinating schedules, supporting caregivers, maintaining compliance, and serving as a primary point of contact for clients and families. This position requires strong communication skills, problem-solving abilities, and a commitment to delivering exceptional service.
Responsibilities
- Client Care Coordination
- Conduct client intake and assist with onboarding new clients.
- Coordinate development and implementation of individualized care plans in collaboration with the Director of Nursing.
- Match caregivers with clients based on skills, experience, and compatibility.
- Maintain regular communication with clients and families to ensure satisfaction and address concerns.
- Monitor service delivery and ensure care is provided according to plan.
- Scheduling & Staffing
- Create and manage caregiver schedules.
- Fill open shifts and address last-minute call-offs.
- Maintain accurate staffing records.
- Monitor overtime and ensure compliance with labor regulations.
- Caregiver Support
- Serve as a point of contact for caregivers regarding schedules, client updates, and performance concerns.
- Assist with onboarding documentation and training coordination.
- Promote a positive and supportive work environment.
- Compliance & Documentation
- Maintain accurate client and employee records.
- Ensure documentation meets state licensing and company standards.
- Assist with audits and regulatory compliance.
- Monitor background checks, certifications, and training renewals.
- Administrative Duties
- Answer phones and respond to inquiries regarding services.
- Assist with billing information, service verification, and payroll coordination.
- Participate in quality assurance initiatives.
Qualifications
- High school diploma required; Associate’s or Bachelor’s degree in healthcare administration, nursing, social work, or related field preferred.
- Minimum 1–2 years of experience in home care, healthcare scheduling, or care coordination.
- Knowledge of home care regulations and compliance requirements preferred.
- Strong organizational and multitasking skills.
- Proficiency in Microsoft Office and scheduling software.
- Excellent communication and interpersonal skills.
- Ability to work in a fast-paced environment and handle urgent staffing needs.
- Ability to pass background check.
