Registered Nurse (RN)

Description

Tata Homecare Services is seeking a compassionate, skilled, and detail-oriented Registered Nurse (RN) to provide clinical oversight and high-quality nursing care to clients in their homes. The RN is responsible for conducting assessments, developing and supervising care plans, ensuring compliance with state regulations, and supporting caregivers to deliver safe and effective services. The ideal candidate is committed to excellence in patient care, strong clinical judgment, and collaborative teamwork.

Responsibilities

• Clinical Assessment & Care Planning
• Conduct initial and ongoing comprehensive client assessments.
• Develop, implement, and update individualized care plans.
• Evaluate client progress and adjust care plans as needed.
• Ensure care is delivered in accordance with physician orders and agency policies.
• Clinical Oversight & Supervision
• Supervise and provide clinical guidance to CNAs, PCAs, and other caregiving staff.
• Conduct supervisory visits in clients’ homes as required by state regulations.
• Provide training and education to caregivers as needed.
• Ensure caregivers are performing duties within their scope of practice.
• Client & Family Communication
• Communicate effectively with clients, families, physicians, and interdisciplinary team members.
• Provide education on disease management, medications, and home safety.
• Address concerns and ensure high levels of client satisfaction.
• Documentation & Compliance
• Maintain accurate and timely clinical documentation.
• Ensure compliance with state licensing regulations and agency policies.
• Participate in quality assurance and performance improvement activities.
• Maintain HIPAA compliance and client confidentiality.
• Administrative Support (if applicable)
• Assist with intake and case management activities.
• Participate in on-call rotation as assigned.
• Support audits and regulatory inspections.

Qualifications

• Current and active Registered Nurse (RN) license in the state of Pennsylvania.
• Minimum 1–2 years of nursing experience (home care experience preferred).
• Current CPR/BLS certification.
• Strong clinical assessment and critical thinking skills.
• Knowledge of home care regulations and standards preferred.
• Ability to work independently and manage multiple cases.
• Valid driver’s license and reliable transportation.
• Ability to pass background check.

• Working Conditions
• Services provided primarily in clients’ homes.
• May require travel within service area.
• Flexible schedule including occasional evenings or weekends.
• On-call responsibilities may be required.
